High Fidelity Wraparound

High Fidelity Wraparound is an intensive care coordination process that helps children, youth, and families navigate complex behavioral health and life challenges. Guided by the family’s vision, voice, and choice, Wraparound brings together natural supports and community partners to create one coordinated plan that reflects each family’s strengths, goals, and priorities.

Rather than working through separate systems alone, children, youth, and families are supported by a collaborative team that coordinates behavioral health, education, healthcare, and other community resources to help them achieve lasting success at home, in school, and in their community.

How Wraparound Supports Families

Every child, youth, and family is unique. High Fidelity Wraparound is designed to build on strengths, honor family voice and choice, and create a plan that reflects what matters most to the family. Through the Wraparound process, families receive support with:
  • Building a team of trusted family members, friends, providers, educators, and other community supports.
  • Creating one coordinated plan based on the family’s goals, strengths, culture, and priorities.
  • Improving communication and collaboration among everyone involved in supporting the child or youth.
  • Connecting with community resources that strengthen family well-being.
  • Developing strategies that help children, youth, and families succeed at home, in school, and in their community.
  • Reviewing progress regularly and adapting the plan as goals and needs evolve.

Who Can Benefit

High Fidelity Wraparound is designed for children, youth, and young adults with complex behavioral health needs who would benefit from coordinated support across multiple areas of their lives.

Wraparound may be appropriate for children, youth, and families who:
  • Are involved with multiple systems such as behavioral health, education, child welfare, or juvenile justice.
  • Need coordinated planning among several providers or agencies.
  • Have experienced significant emotional, behavioral, or family challenges.
  • Would benefit from a strengths-based, family-driven approach to achieving their goals.
Participation is based on eligibility requirements established by New Mexico Medicaid and managed care organizations.

Referral Information

Referrals to High Fidelity Wraparound may be made by healthcare providers, behavioral health professionals, schools, child welfare agencies, juvenile justice partners, community organizations, or families seeking support.

Our team will guide families through the referral process, determine eligibility, and coordinate with the appropriate managed care organization when authorization is required.
